Prime Care Link helps families understand the real options — home care, rehabilitation, residential settings, hospice, equipment, community programs — and helps you take the next step without chasing scattered answers.

  • A hospital says your parent is being discharged and you have a day to figure out where.
  • Someone is coming home and needs nursing, therapy, or wound care there — and you need to know who can actually start.
  • Living alone has stopped being safe, and the words assisted living, board and care, and skilled nursing all sound the same.
  • You are the caregiver now, and nobody handed you a map.

What it costs, and who pays us

Today, our navigation support is offered at no cost to families. Providers do not pay us to be included or recommended, so when we organize options for you, nobody has paid to be on that list. If that ever changes, it is published on this site before it takes effect.

What we don't do

We are not doctors and we do not give medical advice, make diagnoses, or decide what care someone needs. We do not make admission or placement decisions — you and the licensed providers do. We do not handle emergencies. If someone is in danger, call 911.

Getting started

Have these ready, if you can

None of it is required. Call with what you have and we will work from there.

  • Where the person is right now, and any discharge date you have been given
  • The kind of help you think is needed, in your own words
  • Their insurance or coverage, if you know it — Medicare, Medi-Cal, private, or not sure
  • The area you would like care to be in
  • The language the family prefers

In your language

Spanish, Armenian, and Russian are spoken in-house. For other languages we work with you to arrange communication, and every form lets you tell us what the family prefers so the first call is in the right language. Deaf and hard-of-hearing callers can reach us through TTY 711.
