We help organize practical next steps while final clinical, admission, placement, and service decisions remain with the appropriate licensed providers and decision-makers.

How a referral works

A simple, organized process designed to reduce confusion, clarify priorities, and keep communication moving during care transitions.

Start a referral

    Submit
    Start with a short referral form or direct phone call. Share the patient’s basic situation, current setting, discharge timing, location, insurance considerations, and the type of support being requested.

    Review
    We review the information, clarify missing details when needed, and identify the main care-transition priorities. This helps identify which care resource areas may need to be reviewed first.

    Organize options
    We organize available resource options based on care needs, location, timing, family preferences, insurance considerations, and provider availability. Prime Care Link does not make clinical, admission, placement, or service decisions.
  4. 04
    Coordinate next steps
    We help connect the appropriate parties, support communication, and follow up so the referral does not get lost between handoffs. Final decisions remain with the patient, family, legal representative, and licensed providers.
