What we are, and what we are not
Prime Care Link is a care-coordination and resource-navigation service. We help identify and organize options and keep communication moving between the parties. We do not provide clinical care, medical advice, or clinical assessments, and we do not make admission, placement, or service decisions — those remain with the patient, family, legal representative, and licensed providers.
Providers do not pay us to be included or recommended. The options we organize include providers who have no relationship with us at all, and we tell you which is which. How we're paid.
Languages
Every referral form captures the patient's or family's preferred language, and non-English and ASL requests are flagged before follow-up so nobody defaults to a voice-only call to a deaf family. Spanish, Armenian, and Russian are spoken in-house; we work with you to arrange communication in other languages.
