For skilled nursing, home health, hospice, assisted living, board & care, and every other provider
Being listed costs nothing. And it buys nothing.
That is the whole model. Providers do not pay Prime Care Link, so the options we put in front of a family are the ones that fit — not the ones that paid.
What inclusion gets you
- Being included in the options we organize for a family or a discharge planner, when you fit the need
- Being contacted directly when a referral matches what you do and where you do it
- Having your capabilities, languages, and service area described accurately
- Hearing back from us when a referral we sent you has landed — or has not
What it does not get you
- Priority, ranking, or a place at the top of any list
- Exclusivity, or a capped roster that keeps competitors out
- Faster or earlier access to referrals than anyone else
- Anything in exchange for money — there is nothing to buy
Published criteria
Who we include
The same standard for everyone, published here so nobody has to guess.
- Currently licensed by the State of California for the services offered, confirmed at the licensing body's own registry — not a directory
- In good standing at that registry
- Accurate about capacity, languages, and service area
- Responsive: a referral we send you gets an answer, either way
To be included
Contact us with your licence number, the services you offer, the areas you serve, the languages your staff speak, and how you prefer to receive referrals. We verify the licence at the issuing body's registry before anything else.

Written network terms and the formal qualification standard are with counsel. Until they are published here, the criteria above are the standard, inclusion remains free, and nothing is for sale. If that ever changes, it is published on this page before it takes effect.
