Prime Care Link provides care coordination and resource navigation through one trusted point of contact. We help patients, families, and healthcare professionals understand options such as home health, hospice and palliative care, skilled nursing, assisted living and board-and-care placement, wound care, medical equipment, transportation, caregiver support, and related community resources with clarity, discretion, and support. We help coordinate access to these providers, services, and resources; we do not provide clinical care or make clinical decisions.

Our Mission

To reduce confusion during care decisions by giving patients, families, and healthcare professionals a clear, trusted path to appropriate care resources and community support.

From the moment a discharge is being planned through the ongoing coordination of community-based services, Prime Care Link serves as a single, dependable point of contact. We listen, organize available options, and help coordinate practical next steps while respecting each person’s privacy, preferences, and care team.

Our role is not to replace clinical guidance, but to make the navigation process easier, faster, and more understandable.

Our Approach

We listen first because every care situation is different. Medical needs, family dynamics, location, insurance coverage, discharge timing, and personal preferences all affect the path forward.

Prime Care Link reviews each referral, organizes appropriate care resource options, and helps coordinate practical next steps with the patient, family, legal representative, care team, and service providers.

Final care, admission, placement, and service decisions remain with the patient, family, legal representative, and licensed healthcare professionals. Our goal is a smoother care transition with better communication, fewer delays, and less confusion.

SanVel leads Prime Care Link with a clear operating standard: families and care teams should not have to chase scattered answers during a care transition.

As Care Network Director, he serves as the dedicated point of contact for referrals, helping organize care resource options, clarify practical next steps, and keep communication aligned between patients, families, healthcare professionals, legal representatives, and service providers.

His role is to bring structure, discretion, and follow-through to the navigation process — especially when discharge planning, placement needs, home-based support, or community services need to come together quickly. Final clinical, admission, placement, and service decisions remain with the patient, family, legal representative, and licensed healthcare providers.
