  • General care-transition and situational notes
  • Confirmation that the person submitting the referral is authorized to do so

Resource-Guidance Information

When you select “Request guidance” from the Resources page, the selected topic may be placed into the Contact Form message field. That topic is collected only if you submit the form.

Technical Information

Our hosting and technology providers may automatically process standard technical information such as:

  • Internet Protocol address

  • Requested page or URL
  • Date and time of the request
  • Technical error details if the website experiences a malfunction

2. How We Collect Information

3. How Our Forms Work

The Contact Form and Referral Form use your device’s email application to prepare an email addressed to Prime Care Link.

The website does not submit or store completed form information in a Prime Care Link website database. Once you send the email, a copy may remain in your email provider’s Sent folder and in the Prime Care Link receiving mailbox.

Email is not guaranteed to be a secure communication channel. Do not submit medical records, Social Security numbers, medication lists, detailed diagnosis information, medical record numbers, or detailed medical history through these forms.

4. How We Use Information

We may use submitted information to:

  • Review adult referral requests

Prime Care Link helps coordinate access to healthcare providers, care services, placement options, and community resources. We do not provide clinical care, medical advice, clinical assessments, or emergency services, and we do not make clinical, admission, placement, or service decisions.

5. When We May Share Information

We may disclose relevant information to:

  • Prime Care Link personnel who are authorized and need the information to respond or coordinate the request
  • Potential care providers, facilities, or service organizations when the submitter has confirmed authorization and disclosure is reasonably necessary to respond to the request
  • Website hosting, email, security, or technology providers that process information as part of providing their services
  • Government authorities, courts, attorneys, or other parties when required by law or reasonably necessary to protect legal rights, safety, or security

We do not sell or rent personal information.

We do not use personal information for targeted advertising or unrelated third-party marketing.

The current website does not use advertising pixels, behavioral analytics tools, or marketing trackers.

The website does not intentionally set tracking cookies during normal browsing. Hosting and technology providers may maintain standard security, performance, request, and error logs.

Prime Care Link does not track visitors’ online activities over time and across unrelated third-party websites. Because we do not engage in that type of tracking, the website does not respond differently to browser “Do Not Track” signals.

The online Referral Form is limited to adult patients age 18 or older. Do not submit referral information concerning a minor through the online form.

If you believe information about a minor was submitted through the website, contact us so the matter can be reviewed.

Requests are handled manually and may require reasonable verification before action is taken. We will review and respond to requests as required by applicable law.
