7. How Our Forms Work

When you press send on a form, the website sends what you entered as an email to the Prime Care Link inbox. If that cannot happen, the form says so and offers to open your own email program with the message already written, or to copy the text. Share only basic details: no patient names in the Referral Form, a first name or initials at most in other messages, and never medical details, Social Security numbers or other sensitive information. Anything sensitive is better discussed by phone. Our Privacy Policy explains how we handle what you send.
